Thank you for choosing Sojourn Vet for your mobile internal medicine needs. Before your upcoming appointment please be aware of the following: 

Required Documents

Please have your team complete the “Consult Request Form”.  This highlights the intended services to be completed and the chief concerns of the primary veterinarian. This will help to ensure everything is effectively communicated between our team and yours and the maximal benefit of the consult is achieved.

Afterwards, submit the patient’s complete medical records – including all relevant diagnostics and clinician assessments/notes – to: [email protected]

Visit Structure

This appointment can be scheduled as either a stand-alone ultrasound or an ultrasound with a formal IM consultation with the pet owner. For both of these services we welcome owners to be present with their pet as long as your facility allows for it. The visit type must be clarified during the scheduling process to ensure we allot the appropriate amount of time.

When an ultrasound alone is requested, a brief summary of the results and next steps in diagnostics or therapy will be informally discussed with the client, followed by a thorough discussion with the veterinarian. Implementing the next steps and further discussion with the client will then be the responsibility of the primary veterinary team. Reports are submitted at the time of the visit or via email within 24 hours.

If a full consultation is elected during the visit, this includes all aspects of a standard internal medicine consultation (including history from the owner and a physical examination). We will work with your team to ensure all recommended diagnostics are obtained, treatments are prepared, and the pet owners are guided with the next steps moving forward. Additional diagnostics performed will be submitted by your team and results forwarded to us upon completion. This way we can continue to follow up on the case and make additional care recommendations. 

If an in-person visit is not feasible for or elected against by the owner for any reason, we are happy to communicate with them via phone.

Documents For Pet Owners to Review and Sign

All required client forms can be found below. These include the:

  • Client Informational
  • General Consent to Ultrasound
  • Consent for Fine Needle Aspiration

Please send the informational form to the clients as this will go over all necessary preparations for their visit including fasting and use of sedatives if necessary.  All pet owners should complete the “Consent to Ultrasound” form.  If an abdominal ultrasound is being performed, the  “Consent for Fine Needle Aspiration” should be signed ahead of time as well.  Please give these to our team prior to the start of the visit.

Payment

Payment is appreciated at the time of visit. Please let us know whom to contact at your clinic regarding payment prior to the visit. An invoice will be provided to this contact once the visit is completed.
